On-Line Applications

CGFNS now provides applications for all of its programs in Adobe PDF format. In order to read and print the files, you will need to download them to your computer and view them in Adobe Acrobat Reader, Version 5.0. If you do not have Adobe Acrobat Reader, or have an older version than Version 5.0, you can download the latest version from the Adobe web site, free of charge.

Once you have printed an application packet from the internet, please remember to read all instructions very carefully before filling out the forms, neatly and completely. Completed forms and payment need to be mailed to CGFNS.

Certification Program (CP)
First, download the Certification Program Handbook. Carefully read all the instructions contained in the handbook about the Certification Program and how to fill out all of the application forms for the program.

Next, download the Certification Program Applications Packet. Read all instructions very carefully (both on the application forms and in the handbook) before filling out all forms neatly and completely. Once all forms are completed:

1. Forward the Nursing Education Form to the appropriate educational institution(s);
2. Forward all "Request for Validation of Registration/License" forms to the appropriate registration authority(ies); and
3. Send the completed Application (pages 1-3), the Photo Identification Card, the TOEFL Form (if applicable), all enclosures and your payment to: CGFNS, Attn: Certification Program, 3600 Market Street, Suite 400, Philadelphia, PA 19104-2651 USA.

Re-Application for the Certification Program
Download the re-application packet for the Certification Program. Read all instructions very carefully before filling out all forms neatly and completely. Once all forms are completed:
